Registered Nurse, LD Licenses: RN(Compact Accepted)(Copy needed) Certifications: BLS(Copy needed); ACLS(Copy needed); NRP(Copy needed); CERT-Adv. FHM(Copy needed) SSN Required DOB Required References: 1 Reference in entire work history AWHONN cert in fetal heart monitoring
