The Case Manager is responsible for utilization reviews, resource management, discharge planning, treatment plan management and financial management, while also completing medical record documentation.

REQUIREMENTS

Registered Nurse, Case Manager
Experience: 2 years
RN(Compact Accepted)
References: 1 Reference in entire work history
2 years experience in Case Management and Discharge Planning in a Rehab Care setting
