Licenses: RN-NY
Certifications: ACLS(Copy Needed); BLS(Copy Needed); PALS(Copy Needed)

DOB Required
References: 1 Reference in entire work history

1. NY license (#/exp). Ok to submit without an active NY RN
2. Current reference required to send
3. Non local
4. EPIC charting experience
5. Trauma Levels need to be listed for facilities worked at.
